Minnesota Department of Human Rights - SWIFT Certifications portal -  an MDHR Certificate is required for any business that employs 40 or more employees in a state and seeks to enter into a contract for goods or services with the State of Minnesota that is over $100,000 or is likely to exceed $100,000. The Equal Pay Certification is required for any business that enters into contracts with the State of Minnesota for $500,000 or more.

The Location Request form (MS Excel version) should be used when requesting a change to an existing location or requesting a new location to be added for an agency. The location in SWIFT may represent a Bill To Location, Agency Location, Ship To location or Asset Location for an agency. 

The Agency Origin Change Form (MS Excel version) is used when requesting a change to an existing origin or requesting a new origin to be added for an agency. The origin represents the division/district/region for an agency.
