Meeting request
Completed request forms must be received at least 21 days in advance of event date.
To get started:
- Download this form and save it to your desktop.
- Open the saved file and complete all required sections.
- Rename the file using your agency name and the event date.
- Example: mmb-08-16-2022 meeting request.pdf
- Email completed form to SEGIP Meeting Request for review. We will follow up using the contact information you provide within a few days.
- Contact Employee Insurance/SEGIP if your event has not been confirmed within seven days or if you have any questions. Call (651) 355-0100.
