Meeting request

Completed request forms must be received at least 21 days in advance of event date.

To get started:

  1. Download this form and save it to your desktop.
  2. Open the saved file and complete all required sections.
  3. Rename the file using your agency name and the event date.
    • Example: mmb-08-16-2022 meeting request.pdf
  4. Email completed form to SEGIP Meeting Request for review. We will follow up using the contact information you provide within a few days.
  5. Contact Employee Insurance/SEGIP if your event has not been confirmed within seven days or if you have any questions. Call (651) 355-0100.
