• Are at least 18 years old.
• Have a condition such as diabetes, prediabetes, high blood pressure, or abnormally high or low amounts of lipids or lipoproteins in your blood such as bad (LDL) or good (HDL) cholesterol.
• If you smoke or use other tobacco today or have in the past.

Answer a few questions on Omada's welcome page. If you or your family members qualify for the free services, Omada will send you what you need to get started.
If you're eligible, you may only complete Omada's diabetes prevention program once. But, if you get diagnosed with diabetes you may apply for Omada’s diabetes management program even if you've already completed the diabetes prevention program.
