If your spouse or child passes away changes will need to be made to the coverages that they are enrolled in. We are here to help you during this difficult time.
You must make corresponding changes your medical, dental, spouse life, child life, and pre-tax benefits.
Medical and dental coverage
Add or drop medical and dental coverage by submitting the Qualifying Status Change form (Formerly - Application to Change Insurance Coverage).
If you are dropping medical coverage for yourself and you receive the full employer contribution, you must also submit the Waiver of Medical Coverage form and provide acceptable proof of other coverage for you and your eligible dependents.
You cannot change your medical or dental plan administrator.
Life and accidental death and dismemberment coverages
If your spouse died, you must cancel spouse life insurance and accidental death and dismemberment (AD&D).
If your child has died, you must cancel child life insurance if this is your only covered child.
If you have other covered children, you do not have to make any changes to your child life coverage.
Complete an Optional Coverage application to discontinue coverage for either your spouse or child.
SEGIP will submit a death claim to Securian Financial (Minnesota Life) once the Optional Coverage application has been received.
Pre-tax accounts
You may reduce your MDEA and/or DCEA by completing and returning to SEGIP the Change in Participation form.
When you notify SEGIP of the death of your spouse or child, we will submit a death claim to Securian Financial (Minnesota Life) to pay the beneficiary the Life insurance payout as well as any Accidental Death & Dismemberment that may apply.
