Blue Cross and Blue Shield of Minnesota Coordinated Plan

This plan is for eligible SEGIP retirees and their spouses 65 or older.

Blue Cross often updates Coordinated Plan resources in even-numbered years. They explain medical and prescription drug coverage.

How to Find Documents

  1. Go to the Blue Cross SEGIP website.
  2. On the right side of the page, select “Resources for your health plan.”
  3. Scroll down to “Coordinated Plan” where you will find:
    • Group MedicareBlue Rx Group Formulary
    • Group MedicareBlue Rx Summary of Benefits
    • State of Minnesota Coordinated Plan Certificate
    • Other plan details

HealthPartners Medicare Group Solution

This plan is for eligible SEGIP retirees and their spouses 65 or older. Documents on the HealthPartners website explain this medical and prescription drug coverage.

How to Find Documents

  1. Go to the HealthPartners SEGIP website.
  2. At the top of the page, select the "Benefits & coverage" tab.
  3. Select Medicare retirees.
  4. Scroll down to the "Your group Medicare information" section to find "Summary of Benefits PDF."
  5. You will find the list of covered prescription drugs under the "Pharmacy" tab. Select I am a Medicare retiree.

UCare Medicare Group

The UCare plan ended Dec. 31, 2025.

Delta Dental Group 216

This plan is for eligible SEGIP retirees and their spouses. Documents on the Delta Dental website describe the coverage.

How to Find Documents

  1. Go to the Delta Dental SEGIP website.
  2. Scroll down to the "Benefit Summary" section.
  3. Select the State Dental Plan Schedule of Benefits link.
  4. Select the State Dental Plan Summary of Benefits link.

HealthPartners Dental

This plan is for eligible SEGIP retirees and their spouses. Documents on the HealthPartners website describe the coverage.

How to Find Documents

  1. Go to the HealthPartners SEGIP website.
  2. At the top of the page, select the "Benefits & coverage" tab.
  3. Select Medicare retirees.
  4. Scroll down to the "Your dental plan summary" section.
  5. Select "dental schedule of benefits pdf."
