If the Minnesota Department of Human Services makes a decision about payment or authorization of a particular service and you disagree with the decision, you can appeal. You must appeal in writing. You can use the Appeal to State Agency form or write a letter including all the information that the form requires. For more information and help with an appeal, see the Help With an Appeal webpage.
The state appeals office handles appeals about fee-for-service health care, eligibility for health care programs and other issues. More information about the state appeals process is available on the appeals office webpages.
