Drugs - Brand Name Drugs that are preferred over their Generic Equivalent

Authorization will be granted for the generic if the recipient has had a documented:

• Allergic reaction to a specific inactive ingredient in a Brand 
• Adverse reaction to a specific inactive ingredient in a Brand 
• Therapeutic success while taking the generic drug and therapeutic failure while taking the Brand
• Supporting documentation must be provided at the time of request

Third Party Liability (TPL) Authorization will be granted if:

• The recipient has active TPL AND
• Recipient's primary insurance requires the use of the generic
