Home care nursing

Home care nursing services are more extensive than a skilled nurse visit and can include:

• Professional nursing care based on an assessment of the consumer's medical needs
• Ongoing professional nursing observation, monitoring, intervention and evaluation

Who is eligible for home care nursing?

Home care nursing is available to people who are eligible for Medical Assistance or MinnesotaCare Expanded (pregnant women and children) and have nursing needs that are medically necessary, physician ordered, and provided according to a written service plan. Home care nursing services can be provided in the person's home or outside the home when normal life activities take the person outside the home, including school and work.

Home care nursing services can be provided by:

• A registered nurse (RN) or licensed practical nurse (LPN) employed by either a home health agency or PDN Class A licensed agency that are enrolled with Minnesota Health Care Programs
• An independent RN who is enrolled with Minnesota Health Care Programs
• An independent LPN with a Class A license and who is enrolled with Minnesota Health Care Programs
• A parent of a minor child, spouse, non-corporate legal guardian or conservator who is a licensed nurse employed by a home care agency and has an approved HCN hardship waiver.

How can persons get home care nursing?

You can call a home care agency or your county public health nurse to request an assessment for nursing services. To find a home care agency in your area, look in the yellow pages of your local calling area telephone book under Home Health Services.
