IRS Form 1095-B

If you or members of your family were enrolled in Medical Assistance or MinnesotaCare in 2017, the Minnesota Department of Human Services (DHS) will mail you IRS Form 1095-B, Health Coverage. We are mailing the forms throughout January 2018.
This year on your 1040 federal income tax return you will see a question about your 2017 health care coverage. Form 1095-B will help you answer this question.
You do not need to wait to get your Form 1095-B before filing your 2017 federal tax return. If you choose to file your tax return before you get a Form 1095-B, you can answer the tax return question about the months of qualifying coverage received by you and others using your own knowledge or other documents you’ve received as an enrollee. You do not need to attach these documents to your return. Keep them for your records. Other documents you can rely on include the following:
