IRS Form 1095-B
The information on this page is about 2016 coverage and 2016 tax filing.
If you or members of your family were enrolled in Medical Assistance or MinnesotaCare in 2016, the Minnesota Department of Human Services (DHS) will mail you IRS Form 1095-B, Health Coverage. We are mailing the forms throughout February 2017.
This year on your 1040 federal income tax return you will see a question about your 2016 health care coverage. Form 1095-B will help you answer this question.
You do not need to wait to get your Form 1095-B before filing your 2016 federal tax return. If you choose to file your tax return before you get a Form 1095-B, you can answer the tax return question about the months of qualifying coverage received by you and others using your own knowledge or other documents you’ve received as an enrollee. You do not need to attach these documents to your return. Keep them for your records. Other documents you can rely on include the following:
- explanations of benefits
- eligibility notices
