IRS Form 1095-B

The information on this page is about 2018 coverage and 2018 tax filing.

If you or members of your family were enrolled in Medical Assistance or MinnesotaCare in 2018, the Minnesota Department of Human Services (DHS) will mail you an IRS Form 1095-B, Health Coverage. We are mailing the forms throughout February 2019.

On your 1040 federal income tax return for 2018, you need to indicate whether you had full-year health care coverage or an exemption for yourself, your spouse (if filing jointly) and anyone you claim as a dependent. Form 1095-B will help you answer this question. 

You do not need to wait to get your Form 1095-B before filing your 2018 federal tax return. If you choose to file your tax return before you get a Form 1095-B, you can use your own knowledge or other documents you’ve received as an enrollee to indicate whether you had full-year health care coverage or were exempt. You do not need to attach these documents to your return. Keep them for your records. Other documents you can rely on include the following:
