You can submit your Medical Assistance and MinnesotaCare renewal using our online document upload tool.

You can upload your completed and signed renewal form using a scanned file or photos taken with your device. Here’s what to expect:
- You will be asked for some personal information to help identify you and match your uploaded documents with your health care case. Be sure to enter your phone number and email address, if you have one.
- You can upload documents directly from your computer or other device like a phone or tablet.
- You can upload several files at the same time.
- After you submit your documents, you’ll receive a confirmation number letting you know your uploaded files were received. If you provided your email address, you will also receive an email with this confirmation number.
Prepare to upload your form
You'll need:
- Your completed, easy to read, and signed renewal form.
- Clear photos or scanned files of your renewal and any documentation.
- Your case number is not required, however, if it’s not provided, there may be delays in your documents being matched to your case.
NOTE: Your case number is listed on each page of your renewal form.

- Notice of Rights and Responsibilities
- Language Interpretation Page
- Appeals Information
Upload your documents
When you’re ready to upload your documents, choose the submission path based on your household eligibility and follow the instructions provided.
- If you have Medical Assistance, or your household has both Medical Assistance and MinnesotaCare, upload documents to MNbenefits.
- If you have MinnesotaCare and no one in your household has Medical Assistance, upload documents to DHS Health Care Consumer Support Document Portal for Consumers.
NOTE: The upload process may take several minutes for the system to complete. Make sure all files you would like to upload appear in the file list on screen before selecting "Submit." Wait until you receive a confirmation number letting you know your upload was successful before exiting the upload portal.

Your county, Tribal Nation, or DHS Health Care Consumer Support will process your renewal and documents in the order they were received.
- The agency working on your renewal will contact you by mail if more information is required.
- Watch your mail for more information about your case.
If you wish to upload more items that may be requested during the process, return to this site and use the same process to upload any other health care renewal documents as needed.
After your renewal is processed by the agency, you will receive an eligibility notice in the mail.
