Welcome to the ASAM resources page for Minnesota Substance Use Disorder (SUD) treatment providers. This page provides essential tools, training and guidelines to support the implementation of ASAM criteria in assessment, treatment planning and service delivery. Minnesota statute is currently reflective of the of “ASAM Criteria, 3rd Edition” with a goal of implementing “ASAM Criteria, 4th Edition” in the future.
The ASAM Criteria are a comprehensive set of guidelines for placement, continued stay and transfer/discharge of individuals with substance use disorders. These criteria help providers ensure appropriate and effective levels of care.
The “ASAM Criteria, 3rd Edition” covers both adults and adolescents. The “ASAM Criteria, 4th Edition” are divided into two separate manuals: the adult manual, which is currently available, and the adolescent and transitioned-aged youth criteria, which is anticipated to be released early 2026.
ASAM Readiness tool: This self-assessment tool is designed to help providers evaluate their preparedness for implementing ASAM criteria effectively. The tool assists in identifying areas of strength and opportunities for improvement in alignment with ASAM guidelines. This tool is for the agency's benefit and is not intended for review or use by the Minnesota Department of Human Services for any purpose.
The Behavioral Health Administration (BHA) has partnered with Train for Change to provide free ASAM training for SUD treatment providers in Minnesota. These trainings will be available throughout 2025 and will cover various topics, including:
These sessions aim to enhance provider knowledge and application of the ASAM Criteria in treatment settings. E-memos are sent out monthly with the most up to date information for trainings being offered. You can access the e-memos through free e-mail subscription services. Train for Change also offers on-demand ASAM Webinars. Each licensed SUD treatment program in Minnesota is eligible for one training license. A representative from each licensed program can request a training license by contacting us at asam.dhs@state.mn.us.
Acentra
BHA partners with Acentra to implement the federally required utilization management and review processes.
Acentra provides online training, including monthly lunch and learn sessions on ASAM topics and open office hours for assistance with the SUD portal.

Q: Are SUD treatment providers in Minnesota required to use the “ASAM Criteria, 3rd Edition” or “ASAM Criteria, 4th Edition”?
A. Currently, Minnesota statutes are aligned with the “ASAM Criteria, 3rd Edition.” This is reflected in the ASAM levels of care and the ASAM six dimensions outlined in MS 254B. While a licensed program must comply with the requirements in the statute, the program may also integrate “ASAM Criteria, 4th Edition” into the program if desired.
Of note: Utilization Management reviews will be looking for programs to meet “ASAM Criteria, 3rd Edition” requirements.
Q: How do I apply to certify for ASAM 2.5 level of care?
A: Enrolled Minnesota Health Care Programs (MHCP) providers offering substance use disorder outpatient services can apply to offer ASAM level of care 2.5, partial hospitalization, starting Jan. 1, 2025. The Centers for Medicare & Medicaid Services approved the ASAM level of care 2.5, partial hospitalization, for MHCP.
Complete the following steps to comply with DHS requirements to provide ASAM level of care 2.5:
ASAM level of care 2.5 provides intensive, multidisciplinary care of 20 or more hours of skilled treatment services for individuals with substance use and co-occurring disorders. ASAM level of care 2.5 emphasizes evidence-based practices such as comprehensive assessments, individualized treatment plans, integrated care and participation in utilization management reviews. Refer to the “ASAM Criteria, 3rd Edition” for level of care placement criteria for ASAM level of care 2.5.
BHA may contact providers after enrollment to ensure that ASAM level of care 2.5 criteria are appropriately reflected in your provider policies and procedures.
