Best Practices: Care Coordination Conference
The department's Special Needs Purchasing unit hosted an all-day “Best Practices: Care Coordination Conference” for Minnesota Senior Health Options (MSHO) and Minnesota Senior Care Plus (MSC+) care coordinators in 2014. It was funded by the Dual Demonstration grant. Over 400 care coordinators from around the state attended. Some presenters agreed to make their presentation available on the Dual Demonstration webpages indefinitely. Here are links to these approved presentations and handouts.
