Next steps

You received your renewal information in the mail.

Here’s what’s next:

  1. Open it.
  2. Review and complete it.
  3. Return it as soon as possible to avoid delays in coverage.

If you are already enrolled in other health insurance and no longer need your Medical Assistance or MinnesotaCare, call your state, county or tribal agency and let them know.

 
 

When you open your renewal envelope, you’ll either see:

A: Renewal form with information about what to do next.

Read instructions about how to complete the form and how to get help with it if you need it. Make sure your information is complete and accurate.

B: Notice that your insurance automatically renewed.

Make sure your information printed on the notice is correct.

  • If it’s correct, you’re done.
  • If not, write in your corrections and submit the document.
 

Submit your completed renewal form as quickly as possible to your county, tribal or state agency

MAIL: Put documents in the return envelope provided, add postage and put it in the mail. If you don’t have the return envelope, see the County and Tribal directory.

ONLINE: Upload the documents online using the Upload Documents link.

IN PERSON OR FAX: Drop off the documents or fax them to the agency listed as the return address on your paperwork. Addresses and fax numbers for agencies are listed in the County and Tribal directory.

PHONE: Give your information verbally over the phone. Contact your state, county or tribal agency to learn more. Note: Phone renewals may take up to an hour or longer.

Watch for follow-up letters, emails or phone calls

Be aware that you may be contacted for more information if there are questions about the documents you submitted. Continue to report changes.
