MnCHOICES is a web-based application that is comprehensive and integrates assessment and support planning for Minnesotans who need long-term services and supports. MnCHOICES embraces a person-centered approach to ensure services meet each person's strengths, goals, preferences and assessed needs.
MnCHOICES is for people of all ages who have any type of disability or need for long-term services and supports. The MnCHOICES assessment replaces the following tools:
- Developmental disability screening
- Long-term care consultation assessment
- Personal care assistance assessment
- Home care nursing assessment (to be included in the future).

New LTSS Improvement Tool FAQ
DHS recently designed a new way to gather feedback on the recipient experience with long-term services and supports in Minnesota. DHS launched Phase I of the Long-Term Services and Supports (LTSS) Improvement Tool in fall 2017 as part of the MnCHOICES Support Plan application. Case managers and certified assessors will use it to gather feedback from older adults and people with disabilities who receive long-term services and supports.
