Long-term services and supports (LTSS) are a spectrum of services provided to Minnesotans of all ages who have functional limitations. LTSS have the primary purpose of supporting the ability of the person to live or work in the setting of their choice. Living or work settings may include a person’s home, a worksite, a community-based residential setting, such as assisted living, a nursing facility, or other institutional setting, such as a hospital or rehabilitation facility. LTSS enable people to lead meaningful lives at all stages with opportunities to make meaningful contributions and build upon what is important to them.
