Skilled nursing visits
A skilled nursing visit is a nurse visit to initiate and complete a professional nursing task to meet an assessed need in a person's home. A skilled nursing visit may include the following services:
- Hands-on nursing care.
- Health care training for the consumer and/or their family.
- Observation and assessment of the person’s physical status.
Who is eligible for skilled nurse visits?
Skilled nursing visits are available to people who:
- Are eligible for Medical Assistance or MinnesotaCare Expanded (pregnant women and children)
- Have nursing needs that are:
- Medically necessary.
- Ordered by a physician, advanced practice registered nurse or physician’s assistance
- Happen intermittently.
- Provided according to a written service plan.
People may only receive skilled nursing visits in their home.

Who can provide a skilled nurse visit?
Only a registered nurse or licensed practical nurse employed by a Medicare-certified agency may provide this service.
