Resource coordination
Every person has the right to live where they want to live. Minnesota Aging Pathways helps older adults remain in their homes, or return to their home from a facility by providing free in-depth support and follow-up. Minnesota Aging Pathways also provides support to involved family members and caregivers.
Eligibility
Minnesota Aging Pathways helps older adults find the resources they need to stay in their home or move back home from a facility. People are eligible for Resource Coordination if they are:
- Not on Medical Assistance
- In their home and need support, or
- In a facility and wanting to return home

Resource coordination services
Resource coordination begins with a referral, which can come from the person themself, a friend, family member, or provider, or from a referral list provided by the Minnesota Board on Aging comprised of nursing home residents who may benefit from the service.
It begins with a resource coordinator meeting with the person (and their caregiver or family member) to discuss the help that is available. The person then can decide if Resource Coordination is right for them. If it is, together they develop a comprehensive plan that best meet the person's needs and preferences. This includes exploring available resources, estimating costs and setting up needed services, such as:
- Home health
- Meal/grocery delivery
- Transportation
- Help around the house
- Home modification or repair
- Financial or legal assistance
- Caregiver support
- Medicare, and much more
Finally, the resource coordinator provides follow-up to support the person's long-term success in the community.
