Grants and requests for proposals

Statewide partnerships work to expand long-term services and supports, helping older adults remain in their homes and communities of choice. Grant programs provide funding to strengthen these services and support local communities

Grants

Minnesota's Live Well at Home grants provide funding to help communities develop and expand services that enable older adults to remain in their homes and live independently for as long as possible. These grants support innovative solutions, caregiver resources, and programs that reduce reliance on more costly forms of care.
