Manufacturer Reimbursement for Providing Free Insulin

Per Minnesota Statutes 151.74, manufacturers of insulin may submit to the Department of Administration a request for reimbursement for certain charges:

  • Per Subdivision 3(h), manufacturers may request reimbursement of $35 for each 30-day supply of insulin the manufacturer provides for urgent-need insulin.
  • Per Subdivision 6(h), manufacturers may request reimbursement of $105 for each 90-day supply of insulin the manufacturer provides for the continuing safety net program.

By requesting payment, manufacturers agreed to keep all supporting documentation for a minimum of six years and understand that this documentation is subject to audit per Minnesota Statute 16C.05, subd. 5. Per Minnesota Statutes 151.74 Subdivision 11, patient data must be retained as private data and should not be disclosed to the Department of Administration.

Manufacturers may submit multiple requests throughout the year.

  • Requests for manufacturer claims paid out between July 1, 2024 – June 30, 2025, must be received by July 7, 2025.
  • Requests for manufacturer claims paid out between July 1, 2025 – June 30, 2026, must be received by July 6, 2026.

Manufacturers must be registered vendors with the State of Minnesota in order to request reimbursement. Please contact the vendor liaison for registration and submit this form to accountspayable.admfmr@state.mn.us.
