• Investigation Questions & Needed Information: This document lists numerous questions that should possibly be asked during the investigation, as well as other information and documentation that may be needed. Asking the correct questions and obtaining the proper information will help determine the root cause of an accident and also help in the development of good corrective actions. Which questions should be asked and which information and documentation is needed depends on the accident being investigated. Please remember that this document is only a guideline and you may find that other questions, information and/or documentation are necessary.
