- M5 New Vehicle Entry
Use this form to request a new unit be entered into M5 and a Voyager fuel card will be issued.
- M5 Vehicle Department Transfer
Use this form to request a vehicle to be transferred to another department.
- M5 Vehicle Disposal (for agency-owned vehicles)
Use this form to request a vehicle be flagged for disposal, Voyager fuel card will be cancelled, and ARI Holman maintenance will be cancelled (if applicable).
