Safety Contacts | Record Keeping Standard Update: Electronic Reporting of Injury and Illness Data

MNOSHA has adopted the federal rule requiring annual reporting of injury and illness data.

Who Needs to Report

All Minnesota employers with 20 or more employees.

What Needs to be Reported

Required employers must report their 2017 Summary of Work-Related Injuries and Illness (Form 300A) electronically.

When Does Form 300A Need to be Reported

Required employers must report 2017 Form 300A by July 1, 2018.

What Steps are Necessary to Report

  • Ensure your 2017 Form 300A is available and accurate.
  • Proceed to the Injury Tracking Application (ITA) website and review the instructions and specifically the ITA Job Aid section.
  • Launch ITA to begin the reporting process.
  • Click on create an account and follow the steps. An email will be sent to you to create a password.
  • Log into the ITA using the username and password created.
  • Click on create an establishment (the location you are reporting for) and follow the steps to submit your 2017 Form 300A data.
