Safety Contacts | Prep and Post your 2017 OSHA 300A Annual Summary

1/16/2018 2:34:43 PM

  1. Prep your OSHA 300A, the annual Summary of Work-Related Injuries and Illnesses.
  2. Post it in the workplace February 1 through April 30.

Prepping your OSHA 300A

  1. Review your 2017 OSHA log to make sure each injury/illness is correctly documented and the number of days away, restricted, or transfered are correct.
  2. Retrieve your hours worked and average number of employees from SEMA4. MMB has run a job in SEMA4 (HP3603) which produces a blank OSHA300A for each establishment (OSHA location) in SEMA4. Please note: The SEMA4 300As will not have any injury data but will have each Establishments total hours worked and average number of employees. Use the following SEMA4 path to retrieve the report. On-Demand Reports > Standard Report Viewer > DocumentDirecct>HP3603. Be sure to use the January 16, 2018 report. If you don't have access to the report in SEMA4, please check with others within your Human Resources office. If you continue to have difficulty, please contact Todd Christenson at (651) 201-3005.
  3. Retrieve a blank copy of the OSHA300A (second document of three)
  4. Fill in all the fields on the 300A
  5. Have the completed 300A signed by your agency head

Posting your OSHA 300A

  1. You must post a copy of your completed OSHA 300A in each establishment in a conspicuous place or places where notices to employees are customarily posted. You must ensure that the posted annual summary is not altered, defaced, or covered by other material.
  2. You must post your completed OSHA 300A no later than February 1 and keep the posting in place until April 30.
  3. Questions about OSHA Recordkeeping

Please refer to the following resources with your questions about OSHA recordkeeping.

If you are unable to find your answers in the OSHA recordkeeping forms and handbook, you can also contact Kemal Munn at (651) 201-3011.

Thank you for your attention to this State and Federal requirement.
