WuFoo Forms example

This is an example of a wufoo integration. 

You have three options for displaying the form.

1. The user clicks on a button, as seen below, to open the form in a modal window overlay.
2. The form opens in a modal window overlay when the page loads.
3. The form is built into the page and will appear in the page versus a modal window overlay.
