Getting started with Formstack

If you'd like to sign up for Formstack, do not sign up on Formstack's website — we have established a streamlined process for the State of Minnesota that is outlined below:

  1. First decide on which package you'd like from Formstack's list of features. If you'd like a demo, we highly suggest you start by looking over the list of features on their website to ensure it's a good fit for your agency's needs.  If after having reviewed their videos, demonstrations, and documentation you feel like you still need a demonstration, scheduling a demo with a MNIT representative.  In the demonstration, you'll get a short overview on the product and its capabilities, and have time to ask any questions you may still have about the service.  

  2. Sign up for the service.  On the form below, fill out your all the required fields including the name of and email of the person you're designating as your agency's administrator for this service, your contact information, and the package you wish to sign up for.  This will kick off the procurement process involving MNIT's procurement team as well as your agency's Relationship Manager.

  3. Someone from our procurement team will be in touch along with your Relationship Manager (RM).  Your RM will draft up a Service Authorization (SA) for the total amount due and you will be responsible for providing the codes indicating which fund you will be using to pay for the service.  Your first year's cost will be pro-rated from the time your service is set up to the end of the fiscal year (June 30th).  The years that follow you will be billed for the full amount listed on the pricing page.

  4. Once your SA is signed the procurement process begins. MNIT will procure the service through our software vendor SHI, Inc.

  5. Formstack will set up your account and send your administrator an invitation to set up their account.  At that point, we will reach out to you to see if you have any questions and to schedule a short demo of the service to show you how to get started and build your own forms.
 

A few important notes worth mentioning: 

  • You will work with your agency's Relationship Manager to arrange for a Service Authorization to cover the costs of the service.  MNIT will bill you annually.  The first year will be pro-rated from the time your account is established to the end of the fiscal year.  Every year following you will be billed for the full year from July 1 to June 30 of the following year.

  • As you approach the end of the fiscal year, your Relationship Manager will work with you to establish a Service Authorization with MNIT to cover the cost of your renewal.

  • Your account will physically reside under the MNIT parent account, allowing us to provide you direct support with your forms. Basic technical assistance is provided from MNIT's web team to guide you through any questions or issues with the product.

  • You can always add more users, more forms, more storage, and more submissions at any time — reach out to us and we will arrange with Formstack to procure these additional resources and provide you with the cost.  You will be required to sign an SA to cover the pro-rated addition of these features.  Work with your Relationship Manager to arrange for payment.
