Formstack cloud hosted forms service

Updated August 16, 2024 — The State of Minnesota has an enterprise agreement with Formstack, a cloud-hosted forms service that makes it easy to build your own online forms, gather that data via email and in Formstack's cloud database, and a wide array of other options. If you wish to get started with a Formstack account through the State of Minnesota, we've provided most of the information you'll need below.

There are effectively two different packages that we're able to provide access to at a discounted rate:

Annual rate: $3000 per year ($2700 per year including our 10% volume discount)

* The number of submissions is the total number of submissions stored in Formstack's database, not the total number of submissions in the life of the form or per month/year. If you get close to your cap, you simply delete the older submissions in your database to free up more "space" for submissions. If you reach or exceed your submission cap, forms will continue to send you emails when submissions come in (if you created an email based response to send the contents of form submissions to you), but will not be stored in your database.

** The storage space is for files that are submitted via your webforms. As above, removing files from Formstack's database will free up available storage space.

Getting started

If you'd like to sign up for Formstack, do not sign up on Formstack's website — we have established a streamlined process for the State of Minnesota that is outlined below:

First decide on which package you'd like from Formstack's list of features. If you'd like a demo, we highly suggest you start by looking over the list of features on their website to ensure it's a good fit for your agency's needs. If after having reviewed their videos, demonstrations, and documentation you feel like you still need a demonstration, reach out to Michael Tangen (MNIT Central) on the MN.gov web team for a personalized demo. You'll get a short overview on the product and have any questions answered.
Sign up below. Fill out the form at the bottom of this page and that will get you in touch with our Formstack rep to:
- Choose your package option
- Establish a Service Authorization with MNIT to provide payment for the service
- MNIT will work with Formstack to set up your Formstack account and your administrative user account.

A few important mentionables:

You are required to work with your Relationship Manager to arrange for a Service Authorization to cover the costs of the service. MNIT will bill you annually.
Your account will renew at the start of each fiscal year. As you approach the end of the fiscal year, work with your Relationship Manager to establish an SA with MNIT to cover the cost of your renewal.
Your account will physically reside under the MNIT parent account, allowing us to provide you direct support with your forms. Basic technical assistance is provided from MN.IT's web team to guide you through any questions or issues with the product.
You can always add more users, more forms, more storage, and more submissions at any time — reach out to us and we will arrange with Formstack to procure these additional resources and provide you with the cost. You will be required to provide an SA to cover the pro-rated addition of these features. Work with your Relationship Manager to arrange for payment.

Information for Purchasing Managers

Your agency will be billed annually by MNIT, and is responsible for working with your Relationship Manager to draft up the Service Authorization for the services you've signed up for. Have your chargeback number and other budgetary information ready and on-hand. MNIT will provide you with the cost of the service so that you can make the appropriate budgetary decisions. MNIT will collect the funds from you and MNIT will remit both the initial payment and ongoing renewals through SHI / invoicing with Formstack. ALL PREVIOUS BILLING METHODS ARE BEING REPLACED. Those that were working directly with Forrmstack to remit payment will now work with MNIT and your Relationship Manager to draft the necessary Service Authorization and payment.
