Revalidation is a mandatory, periodic process to verify that provider enrollment information, credentials, and practice details are accurate and compliant with state and federal regulations. This process entails credential verification and may require unannounced site visits, and background checks depending on the risk levels of the Medicaid services you provide. It is a measure that helps combat fraud, increase program integrity, and safeguard Medicaid funding.

Off-cycle revalidation is a revalidation of a provider’s enrollment that occurs outside the standard periodic revalidation cycle (every 5 years for most providers and every three years for certain designated providers.) The new off-cycle revalidation effort, also known as Minnesota Revalidate 2026, is in-place because of orders from the federal Centers for Medicare and Medicaid Services, which instructed the department to revalidate all providers delivering high-risk benefits and services by May 31, 2026.

It launched on Jan. 26, 2026.

Revalidation notices were sent by U.S. mail and the department’s secure email system for enrolled providers. Providers also received messages via subscription email updates and the department’s provider news and updates webpage. Providers who received a revalidation notice prior to January 26, 2026, are still required to complete the revalidation process. Providers who were previously screened at high risk and successfully completed an unannounced site visit in 2025 did not receive a revalidation notice.

Following Executive Order 25-10: Empowering State Agencies to Continue Combatting Fraud, Governor Walz announced a pause on payments for 14 identified high-risk Medicaid services. The pause allows for a payment integrity audit, also referred to as pre-payment review, to be conducted on claims submitted in these service areas. The original 14 high-risk services (now 13 due to the discontinuation of Housing Stabilization Services):

4. Assertive Community Treatment 
5. Community First Services and Supports 
6. Early Intensive Developmental and Behavioral Intervention 
7. Housing Stabilization Services (discontinued)
8. Individualized Home Supports
9. Integrated Community Supports 
10. Intensive Residential Treatment Services
11. Night Supervision Services
12. Nonemergency Medical Transportation Services 
13. Recovery Peer Support 
14. Recuperative Care

The “high-risk” label does not mean that a specific provider is more likely to engage in fraud, waste or abuse. These program types were identified by Governor Walz as having structural risks that make them vulnerable to fraud, waste or abuse. By designating them as high risk, the department is able to more efficiently oversee these programs to detect and eliminate fraud, waste and abuse.

The pre-payment review process resulting from a directive in the Governor’s Executive Order 25-10 (directive 1c, specifically), audits provider claims to ensure that service delivery requirements for Medicaid payment have been satisfied. Off-cycle revalidation, implemented in response to orders from the federal government, revalidates a provider’s credentials to be actively enrolled with Minnesota Health Care Programs.

This will not have an immediate impact on where enrollees can receive care, but if a provider does not successfully complete their revalidation by May 31, 2026, the provider will be terminated from Minnesota Health Care Programs.

There should be no impact on claim reimbursement due to the revalidation process, if providers are able to successfully revalidate. If providers do not successfully complete revalidation by May 31, 2026, they will be terminated from Minnesota Health Care Programs.

May 31, 2026, is the deadline for all impacted providers to complete their revalidation.

Providers who do not revalidate by May 31, 2026, will be terminated from Minnesota Health Care Programs.
