Opiate Product Registration Fee Assessment Dispute


Instructions for Filing

A manufacturer may dispute the Board of Pharmacy’s determination, that the manufacturer must pay the product registration fee, no later than 30 days after the date of notification.

However, the manufacturer must still remit the fee as required by Minnesota Statutes, Section 151.252, subdivision 1, paragraph (b).

The dispute must be filed using the Opiate Product Registration Fee Dispute Form and must include any additional documentation to demonstrate that the assessment of the registration fee was incorrect

To file a dispute, download and complete the information requested on the Opiate Product Registration Fee Dispute Form, sign and date. The completed form may be submitted by any of the following methods:
