Professional Firm

Please read the following statute regarding professional firms.

New Applicant

To apply please contact the Board for the Professional Firm Application.

This application is for new in-state and out-of-state professional firms. No statute or rule requires a pharmacist or pharmacy to register as a professional firm in order to provide pharmacy services in Minnesota. However, individuals may choose to register a professional firm.

Renewal

To renew use the Professional Firm Renewal Application. Renewals are due January 1 of each year.
