The documents available on this page will list most of the disciplinary actions taken by the Board from 2001 to August 2023. Please note that they do not contain all of the disciplinary actions taken by the Board during this period of time. To verify whether or not a disciplinary action has been taken against a licensee or registrant, please use the Board’s license verification system. If that system indicates that a disciplinary action has been taken against a licensee or registrant but does not have corresponding documentation available, you may request the public information by sending an email to pharmacy.board@state.mn.us. Use our general search feature to determine which of these documents, if any, contain the name of a specific licensee or registrant that you are looking for. For example, if you enter "John Doe" in the search field and click search, any document on the Board Web site that contains "John Doe" will be displayed; including any of the disciplinary documents that contain that person's name.
