Any pharmacist who has been licensed by the board and has defaulted in the payment of the renewal fee may be reinstated within two years of such default without examination, upon payment of the arrears, and upon compliance with the provisions of Minnesota Statutes Section 151.13, Subdivision 2.

Once you have reviewed the Reinstatement Standards and submitted the application, the Board will review your application and contact you with specific requirements and payment due within 10 days of receiving your application. Applications can be emailed, faxed, or mailed to the Board.
