Free and low-cost care
Advantage Value for Diabetes ensures you can more easily afford medications and the necessary diabetes-related care. You get automatically enrolled in the benefit when Blue Cross or HealthPartners get your medical bill for care connected to diabetes. Here’s how the benefit helps you:- No cost to you for diabetes-related and dietitian office visits if the primary care clinic (PCC) you selected is in cost level 1 or 2. If your clinic is cost level 3, your copay is $35; cost level 4 clinic copays are $55. You do not need to pay your deductible before insurance pays for your diabetes care.
- No cost to you for diabetes retinal eye exams.
- No cost to you for lab tests for cholesterol, urine protein, glucose, and Hemoglobin A1C.
- No cost to you for eligible supplies such as test strips, insulin pump supplies, syringes, lancets, and more when you buy from CVS Caremark network pharmacy and use your prescription ID card. (If you buy supplies somewhere else using your HealthPartners or Blue Cross ID card, you will pay 10% of the cost.)
- Lower costs or no cost for medications that treat diabetes, high blood pressure, cholesterol, and depression.
