Sign Up for the Vaccine Connector
The Minnesota COVID-19 Vaccine Connector is a tool that helps Minnesotans find out when, where, and how to get a COVID-19 vaccine. After you fill out a simple form, the Vaccine Connector will connect you to resources to schedule a vaccine appointment and notify you if there are vaccine opportunities in your area.

If you have questions about the Vaccine Connector, need assistance filling it out, or need to request Americans with Disabilities Act (ADA) accommodations, please call the Minnesota Department of Health COVID-19 Public Hotline at 1-833-431-2053 between 9:00 a.m. to 7:00 p.m. Monday-Friday and 10:00 a.m. to 6:00 p.m. on Saturday.
Who should sign up for the Minnesota COVID-19 Vaccine Connector?
- All Minnesotans who have not yet been vaccinated should sign up!
Why should I sign up?
- The Vaccine Connector will notify you when there are opportunities for you to get the vaccine.
- Vaccine Connector users may also be selected to make an appointment at one of the state’s Community Vaccination Program locations. If you are selected, you will be notified by text, email, or phone call by one of the state vaccination partners running those sites, Vault Health, Primary Bio or SpeciaLysts (Solv). Additional community vaccination locations may launch as the federal government makes more vaccines available in the future.
