Get support managing or preventing diabetes

Diabetes is one of the fastest growing diseases in the U.S. While there are actions people you take to help manage or prevent the chronic disease, it can require a lot of careful attention. That’s why our health plan offers a free program that can help you and your adult covered dependents. 

The program is called Omada; and it provides personalized tools and assistance to help you reach your health goals. Minnesota Advantage Plan members have been able to use Omada for diabetes prevention, and now it’s expanded to include diabetes management. 

Through Omada, you have access to:

  • A personalized health program based on your goals
  • A health coach to keep you on track
  • Devices, such as a smart scale and/or glucose meter
  • An online community tailored to your interests

Get started

To get started, visit the Omada page and take a short questionnaire to find out if you or one of your dependents are eligible. Individuals who did not previously qualify may now be eligible with the expanded benefits.

Don’t forget! If you or your family members has diabetes, you also automatically receive the Advantage Value for Diabetes benefit. This can help lower your out-of-pocket costs for high-value services, prescriptions, and testing supplies. Learn more.

Earn well-being points through Omada 

Each week when you complete all three milestones in Omada, you can earn 25 points toward our well-being program. Milestones include a weigh-in or glucose monitoring, meal tracking, and a lesson. You can complete this activity up to six times, earning up to 150 points. When you earn 300 points by Oct. 31, 2021, you’ll get $70 off your individual deductible next year. (Eligible employees only.)
