Already Scheduled Your Short-term Leave?

Short-term disability insurance helps replace employee income while they are out of work. Employees who are pregnant or have surgery scheduled for early 2025 may have a short-term disability claim already in process and will want to take these steps to transition easily from The Hartford to MetLife.

If Your Leave Begins As Scheduled in 2025

Contact your Human Resources office. They'll help you:

  • Get a MetLife claim number. You may call MetLife at 833-622-0135 to get started.  
  • Submit paperwork to MetLife. If you've already submitted paperwork to The Hartford, you may submit the same paperwork to MetLife. MetLife will contact you if they need any more information. 
  • Complete and submit a form that gives MetLife permission to receive medical information about you.
  • Register on the MetLife MyBenefits website on the State of Minnesota page.
    • You'll need the employee ID you use to log in to Self Service during open enrollment. You may also use your social security number.
      • You may also go to mybenefits.metlife.com to register. This is not a SEGIP-specific page. 
      • Go to the Access MyBenefits box. Type and select State of Minnesota, then register.
  • If you have questions, call MetLife at 833-622-0135.

If Your Leave Begins in 2024

From now through the end of 2024, you will work with The Hartford/Ochs. You have already received the information you need to work with The Hartford/Ochs. If your leave begins on or before Dec. 31, 2024 you will stay with The Hartford until your leave ends and the claim process is finished.

Only the disability insurance vendor is changing.

The benefit, enrollment, and eligibility rules remain as they are now. 

Life insurance coverage remains with Securian.

Securian will launch a Minnesota-specific website soon. If you have questions now, contact Securian at 1-866-293-6047.
