Changing Plan Administrators

Moving to Blue Cross or HealthPartners

What changes, what doesn’t, and what you need to do.


  • Confirm your current specialists, therapists, or other providers are in your new network. 
  • Call before you seek care. Make sure all your providers are in your new network for 2024.
  • Ask about “continuity of care” services if find your current specialist or other provider is not in your new administrator’s network. You may be able to keep seeing your provider(s) for a while. Not all situations qualify for continuity of care services, so be ready to talk about the care you need and when you need it. If you need to find a new provider, your plan administrator can help find one who meets your needs.
  • Make sure Blue Cross or HealthPartners know about any referral you have for care in 2024. Most medical services you get outside your primary care clinic require a PCC's referral. If you have a referral for care in 2024, contact your new plan administrator. You may also need to contact your clinic for a new referral. 

Continuing Current Programs and Support

Blue Cross and HealthPartners are working directly with PreferredOne to make your transition as smooth as possible. Both Blue Cross and HealthPartners will be supporting former PreferredOne members who have had prior authorizations, enrolled in disease management programs, or received other support. Contact your 2024 plan administrator if you have any questions about the transition or the support programs they offer.
