Employees and their family members enrolled in the Minnesota Advantage Health Plan (Advantage) can get free over-the-counter COVID-19 home tests. Select one of the options below to make sure your tests are paid for.
You may get up to eight tests every 30 days for each family member who is in Advantage, the state’s employee health insurance. If tests are in short supply, the pharmacy may limit the number of tests you may purchase at one time.
To ensure the cost of your test is covered, there are two options:
Option 1 No payment required at in-network pharmacy: Present your CVS prescription benefit ID card to any in network pharmacy that offers COVID-19 tests and have no out-of-pocket cost. To find participating pharmacies, sign in to caremark.com and use the pharmacy locator.
Option 2 Buy the tests and get reimbursed up to $12 per test. If you buy tests online or from a store that is not an in-network pharmacy, follow this process for getting reimbursed:
If you do not have a caremark.com account, register online.
If you have questions regarding coverage, check out CVS Caremark’s FAQs or call CVS Caremark Customer Care at 1-844-345-3234.
Note: Reimbursement is from CVS Caremark. Minnesota Advantage Health Plan’s administrators, BlueCross BlueShield, HealthPartners or PreferredOne, won’t be able to help you get reimbursed.
