The State will provide a copy of Form 1095-B or 1095-C to the IRS by the March 2 deadline. However, it's a good idea to keep these records on hand to verify coverage.
IRS Form 1095-C is for employees that worked full-time or received health coverage through their state employment at any point during tax year 2020. Sign up to get your form electronically to get faster access. Go to Self Service, select Benefits, and then 1095-C Consent.
If you signed up to get your form online by Tuesday, Feb. 9, you will be able to access it electronically in Self Service by March 2. If you did not, we will mail your form to your home address in mid-March.

IRS Form 1095-B is for retired and former employees who paid premiums for State of Minnesota (SEGIP) insurance directly to the health plan and/or had a balance in the Health Care Savings Plan (HCSP).
This form is available by request only; you will not receive a copy unless you request it. Request a copy of your 1095-B forms by contacting Minnesota Management and Budget. Use our convenient online form or send us a request by mail. For more information or questions, call us at 651-355-0100.
