The Affordable Care Act (ACA) requires nearly all Americans to have health insurance. Issued by the State of Minnesota as your employer, the 1095-C serves as proof that you had coverage. The State will send a copy of your form to the IRS later this spring.

1095-C forms are now available for tax year 2017. If you opted to receive the form electronically, it is now posted to Self Service. For all other employees, the forms were mailed by February 1, 2018.

Keep this form with your other tax documents. You do not submit the form as part of your personal income tax filing, though your tax preparer may request a copy. The State of Minnesota will send a copy to the IRS on your behalf.
