The State of Minnesota provides employees with an IRS tax form 1095-C if they worked full-time or received minimum essential health coverage through their state employment at any point during tax year 2016.

For employees who opted to receive this form electronically by January 25, 2017: it is now available in Self-Service. To download yours, go to Self Service > Benefits > View Form 1095-C.

For everyone else: The form was mailed to your home on Friday, February 3, 2017. It will also be posted in Self Service by late spring.
