The State of Minnesota provides employees with an IRS tax form 1095-C if they worked full-time or received health coverage through their state employment at any point during tax year 2016.
MMB will mail these forms by March 2, 2017. For employees who opted to receive this form electronically instead, it will be posted to Self-Service at least two weeks prior. We will update our website once they are available.
