The State will provide the IRS a copy of Form 1095-B or 1095-C by the deadline. Although the state is required to provide these forms, you do not need them to file your taxes.
For Employees: IRS Form 1095-C
IRS Form 1095-C is for employees who worked full-time or received health coverage through their state employment at any point during tax year. You may find this form in Self Service. Select Benefits, then Insurance. The form will be listed on the left side of the page.
If you did not consent to receive your 1095-C electronically, we will mail a hard copy to your home address in mid-March. To receive it in Self Service in the future, select Benefits, then Insurance—the 1095-C Consent form is on the left side of the page.

For Retired or Former Employees: IRS Form 1095-B by Request Only
IRS Form 1095-B is for retired and former employees who paid premiums for State of Minnesota (SEGIP) insurance directly to the health plan and/or had a balance in the Health Care Savings Plan (HCSP) administered through Minnesota State Retirement System (MSRS).
This form is available by request only and will not be mailed to your home unless specifically requested. You can request a 1095-B form by contacting Minnesota Management and Budget. Use the online form, send a request by mail, or call 651-355-0100.
