All IRS Forms 1095-B and 1095-C for reporting on tax year 2016 have been issued.

The Affordable Care Act (ACA) requires nearly all Americans to have health insurance. The 1095 serves as proof to the IRS that you had coverage. The State of Minnesota issues both 1095-B and 1095-Cs. A copy of your form will be sent to the IRS by the due date.

The 1095-Bs were mailed on February 25, 2017 to the most recent home addresses we have on file for you. You will receive this form if you are a former employee who is under age 65 and during any month of 2016 you:

  • Purchased health care insurance through the State of Minnesota (SEGIP) and paid your premiums directly to the health plan; and/or
  • Had a balance in your Health Care Savings Plan (HCSP) administered through Minnesota State Retirement System (MSRS).

The 1095-Cs are issued to an active state employee who during any month of 2016 was full-time or had health coverage through SEGIP. It is also provided to former employees who received an employer contribution or purchase COBRA through SEGIP during any month of 2016. These forms:

  • Were posted in Self Service on January 25, 2017 for those who agreed to electronic delivery.
  • Were mailed to all others on February 3, 2017 at the most recent home address listed in Self Service.
