Workforce Certificate Application Form

The application form requires contractors to submit company information, complete the Company Agreement, and provide an online payment confirmation number.

Compliance Plan

Companies must submit a Compliance Plan and are encouraged to use the template below. Contractors are not required to use the Minnesota Department of Human Rights' template but must provide the required information to obtain a certificate.

Data Analysis

The Workforce and Utilization Analysis (WUA) is snapshot of your current workforce by job group. The Availability and Underutilization Analysis (AUA) compares your current workforce to the available percentage of employees in applicable job groups and labor markets.

Optional Worksheets

State in the WUA if any of the sheets are used to determine availability percentages.

• Composite Availability Calculator (Excel): Submitted if the business chooses to use multiple Standard Occupational Classification (SOC)/Census Occupational Code (COC) codes in one job group in lieu of a single US Census identified SOC/COC code to determine availability due to multiple job titles in a given job group.

• Two-Factor Analysis (Excel): Submitted if a business chooses to determine its utilization based on internal staff availability to train for and perform the job and external workforce availability. These two data are weighed to determine workforce availability. 

Census Data

The Availability and Underutilization Analysis and the Annual Compliance Report must use data from the 2018 Census.

Online Payment

The fee for a Workforce Certificate is $250. You can pay online by using a credit card or a check by using the routing number and checking or savings account number.
