Ban the Box Submission Form
The Minnesota Department of Human Rights is charged with enforcing the Ban the Box law for private employers. It requires employers to wait until the applicant has been selected for an interview, or until a conditional job offer has been extended, before inquiring about the applicant's criminal history.
Our Department reviews employment applications submitted to us, through various means. If the application includes violations of the Ban the Box law, we work with the employer to bring the application into compliance.
You can submit employment applications to us for review through the form below or by emailing BanTheBox.MDHR@state.mn.us.
Submissions can be submitted confidentially. However, our Department will not be able to contact you if we have a question about your submission if you do not submit your contact information. The Minnesota Department of Human Rights does not share individuals’ contact information with employers.
