ALD License Renewal Process for 2022-2023

Important Information!

  • Online Licenses renewals will be available from September 1st to October 31 and late online renewals from November 1 to  December 31 2022.
  • All non-renewed Assisted Living Director licenses expire on November 1st 2022. If you are an Director of Record, you must have an active license to operate a Minnesota Assisted Living Facility on November 1, 2022.
  • If you are not planning to renew, please complete the online form. The ALD Non-Renewal Form can be found here: https://forms.gle/87zTCDe9sZZwuPCo6 . You will continue to receive emails until October 30th 2022.
  • Licenses will be mailed out within 5-8 business days after a licensee renews. If your license hasn't shown within that time period please fill out the Lost License form. Form Link: https://forms.gle/X7yBiN44fSbkDhpP6
  • As a reminder, the renewal of license now requires a clock hour attestation. In lieu of entering your clock hours on the website, you maintain your own personal file of CE and only respond an audit request, validating your attendance.
