Mentor Resources

The purpose of a mentor is to ensure the ALD applicant or ALDIR complies with the domains of practice and the NAB training module while applying the experience with professional practice analysis.  The field experience should include 1) exposure to all areas of operations within an assisted living 2) review findings and results of regulatory inspections and responses 3) observation of the integrative and administrative role of the director of record through various meetings with internal and external stakeholders 4) Observation of the relationships between the facility and community partners in the continuum of health care 5) participation in quality assurance and performance improvement projects. The mentor will help determine the duration of the field experience based on the topics identified in the self-assessment completed by the ALD Applicant/ALDIR.  At the end of the field experience, an ALD Applicant/ALDIR and Mentor must provide evidence to the board completion of the self-assessment and remediation of any areas identified as deficient through the mentor evaluation process.

The Mentor Evaluation Form listed below will prove completion of the self-assessment and field experience.
