Education Review

To complete the request for education and experience review:

  1. Download the Education and Experience Review form --Note: it is important to review this form thoroughly prior to submission.

  2. Gather the following to submit with the education and experience review form:
    • Transcript showing at a minimum a Bachelor's Degree.
    • Course descriptions/syllabi for each academic course submitted on the review.
    • If your education was completed over 7 years ago, submit the following:
      1) Copies of continuing education certificates or work experience.

  3. If you are requesting a reduction or waiver of practicum, submit:
    • Resume (since college)
    • Employment verification, organizational chart, and position description from your most recent employer.

  4. Next steps in the education and experience review process:
    • An educational and experience review is not an application for licensure.  For those requesting a review prior to opening an application, the review fee is $100.  
    • Mail your completed education review and experience form to the address listed on the form.  You will be mailed a determination to help you decide if you would like to open a Nursing Home Administrator (NHA) application.  
    • Once the determination is made, if you would like to proceed with converting your review to an NHA application, further instructions and the full application will be sent to you by email.  You will need to submit the completed application and additional application fee within 12 months of the initial education and experience review form being submitted.

Please note: Education and experience reviews are only good for 12-months.
