All replacement documents bear the word Replacement and include the date that the replacement document was issued.
Replacement Licenses
If your original license document has been lost, stolen, or destroyed, you are required to report the loss to the Board. If you want a replacement license submit the request for document replacement form with a fee of $20.00.
Replacement Public Health Nurse Certificates
If you wish to receive a replacement public health nurse registration certificate, submit the request for document replacement form with a fee of $20.00. Include name change documentation materials if your name has changed.
Replacement Permits
The Board does not issue replacement permits.
